Directions for online Forms
We now have online forms for our adult clients!
To complete your intake paperwork, please follow the directions below:
1. Obtain your username and password from your clinician.
2. Go to: https://br.carepaths.com/signin
3. Enter your username and password
4. Reset your password and click “Save”
5. Hit the “Home” in the upper left-hand corner
6. Click the “Clinical Documents” button
7. Select the “Santa Fe Adult Intake*” link
8. Fill out the form and type your name in each signature box
· You can save as you go by clicking the blue “Save” button at the bottom
· To complete the document go the bottom and enter your password and click the “Sign” button
· All signature areas must be completed in order to sign
*Forms for child clients will still need to be emailed to you.
